DATE VA MAILED THE NOTIFICATION OF THE DECISION TO THE CLAIMANT.

Here is a copy of the new VA NOD form, 21-0958 I received today from VARO Detroit.   Under Secretary Hickey mentioned this in her talk at CAVC.

 

I could not find it on the VA.Gov site so it is not in fillable format.
